Stop Calling the Payer

The 276 asks a payer for the status of a submitted claim; the 277 answers. It replaces a phone call. The frequent confusion is with the 277CA (Claim Acknowledgment), which is a different transaction reporting whether a claim was accepted for adjudication in the first place.

277 and 277CA are different transactions

The 277 responds to a 276 inquiry about a claim in adjudication. The 277CA is unsolicited and reports the payer's front-end edit result: accepted or rejected for adjudication.

A claim rejected at 277CA will never produce a meaningful 277 status, because it is not in the queue. Organizations that poll status on claims that were never accepted are asking a question with no answer.

Polling is a symptom

Heavy 276 volume usually indicates the acknowledgment chain isn't being consumed. If you know your 999 and 277CA results, you know which claims are actually in adjudication and roughly when to expect an 835 (the Electronic Remittance Advice).

Status inquiry then becomes an exception tool rather than a daily sweep.
