The 275 carries clinical documentation supporting a claim or prior authorization, linked to its parent by control number. It exists, it works, and it is underused, because payers differ on what they accept, when, and how they want it linked.

If the control number doesn't match what the payer expects, the documentation lands unassociated. The claim then denies for missing documentation that was sent. Both parties did their part; the result is a denial.

This is the most common 275 failure and the hardest to detect from the sending side.
